Analysis of Neurology Inpatient Re-admissions, and Identification of Areas for Improvement

Rationale: The goal of this study is to identify the cause of re-admissions to the Neurology Inpatient Service at our community hospital over a 12-month period, and to assess whether these re-admissions were preventable. Analyzing these re-admissions may identify our most vulnerable patients and suggest areas for improvement in their care. _x000D_
Methods: We retrieved hospitalization data of all admissions to the neurology service over a12 month period (June 15, 2020 – June 15, 2021). We identified all patients with re-admissions over a 90-day period. We reviewed their medical records for diagnosis, discharge plans and the cause of re-admission. Reviewing the charts, we searched for any preventable causes of readmission in these patients._x000D_
Results: Over 12 months, there were 949 admissions to the Neurology Inpatient Service at our community hospital. Among these admissions, there were 53 (5.58%) re-admissions within a 90-day period. The most common original admission diagnosis in the patient who were then readmitted was ischemic stroke (n=13, 28.3%), followed by seizure (n=11, 23.9%) and intra-cerebral hemorrhage (n=7, 15.2%). The most common re-admission diagnosis was seizure (n=13, 28.3%), followed by ischemic stroke (n=12, 26%) and ICH (n=7, 15.2%). Among patients who were readmitted with seizure, non-compliance with medications was the single most common preventable cause (56% of patients)._x000D_
Conclusions: The most common cause for 90-day readmissions in neurology is seizure, and medication non-compliance is the most common preventable cause of seizure re-admissions. At our community hospital, there is a need to improve community support and early follow up for epilepsy patients. _x000D_
