Rationale:
Medically refractory epilepsy (MRE) is defined as failure to achieve seizure freedom despite a trial of two appropriately dosed anti-seizure medications (ASM). In such patients, the likelihood of failing a third ASM is up to 95% rendering them candidates for pre-surgical evaluation. Often in such circumstances, no further medication adjustments are made until invasive EEG monitoring or epilepsy surgery has been completed. In this study, we report a cohort of patients who underwent pre-surgical workup, were deemed appropriate surgical candidates, and subsequently achieved seizure freedom prior to surgical intervention with medication adjustment alone.
