Rationale: The ketogenic diet (KD) is a well-established intervention for children with refractory epilepsy. There is growing evidence for the use of KD in the intensive care unit (ICU) for children and adults with super refractory status epilepticus (SRSE). While most case series focus on KD for SRSE, emergent KD initiation is not necessarily limited to those with SRSE. Here, we present the largest case series of patients initiated on the KD in the ICU setting for any reason and assess safety, efficacy, and long-term follow up. Methods: All consecutive patients started on the KD in the pediatric and neonatal ICU at Children’s Hospital of Philadelphia from 2010-2018 were included. Medical records were reviewed for demographic, laboratory, EEG, and imaging data. Data were summarized using descriptive statistics. Ketosis was defined as serum beta-hydroxybutyrate level ≥1.5mmol/L, acidosis as CO2 ≤17mg/dL, mild hypoglycemia as glucose <60mg/dl, and severe hypoglycemia as glucose ≤45mg/dl.
