Rationale: The purpose of this study was to compare the difference in the spectral and spatial distribution of the source between two initial ictal EEG patterns (type1 vs. type 2, Ebersole and Pacia, 1996) in surgically proven HS-MTLE using LORETA. Methods: EEG data of 25 scalp electrodes with Pz reference were obtained from pre-surgical EEG monitoring in 18 patients (type 1 vs 2=9:9). EEG data were fragmented in consecutive epochs of 1s at initial ictal rhythm and baseline awaking EEG, respectively. Ictal onset on the right side was flipped to left side.
