To determine the difference in seizure semiology over time and its relation to surgical intervention., We analyzed 20 patients (11 male, 35[plusmn]15 years old) who underwent ictal video-EEG monitoring at least two different times. The minimum interval between two monitoring periods was one year, while the maximum time lag was 13 years. Thirteen patients had resective surgery between the two monitoring periods, while the remaining seven patients had no operation. We analyzed the difference between the two monitoring periods according to seizure types, presence of aura, presence of secondarily generalized seizures, duration of seizures and presence of lateralizing signs., In the non-surgical group we did not find any significant changes in seizure semiology. Patients who underwent resective surgery between the two ictal recordings showed no significant difference in seizure types, seizure duration, or secondary generalization. Ictal head version occurred in 62% in ictal recordings performed prior to surgery, while this appeared in only 23% in ictal recordings captured postoperatively (p[lt]0.05)., Seizure semiology in adult patients shows no clear changes over time. In those patients who underwent a surgical procedure, ictal semiology shows only a minor difference.,
